Directions:
We are located on the northeast corner of Route 108 and Route 100 off of Snowden River Parkway. In our parking lot, there are three multi-story glass and concrete buildings that back to Route 100. As you enter the parking lot, we are in the five story building straight ahead of you.
From North of Columbia
Take Route 29 South to Route 100 East. Take to Exit 3 (Snowden River Pkwy).
Turn left at the first light onto Waterloo Road.
Make an immediate left into Columbia Corporate Park 100.
University Boulevard is the parking lot street.
We are in the building straight ahead of you on the third floor.
From South of Columbia
Take Route 29 North to Route 175.
Go to Snowden River Parkway northbound.
Take this to Waterloo Road and turn right.
Take your first left into Columbia Corporate Park 100.

Cancellation Policy:
If you are unable to keep your appointment, kindly provide a minimum of one business day notice to avoid being assessed a cancellation fee. This fee ranges from $25 to $100 depending on the nature of the appointment. All cancellations for cosmetic and surgical procedures will be assessed $100 if adequate notice is not provided. Appointments can be cancelled by calling our office 24-hours a day.
Prescription Refill Request:
We are happy to assist you with requests to refill your prescriptions. There are two ways to proceed. Your dermatologist may have designated a specific number of refills at the time the prescription was originally written or you may need a new prescription.
If you have refills left on your prescription:
Call your pharmacy and provide them with the reference number on your prescription bottle. They will be able to refill your prescription. This is the most efficient way to receive your medication.
If you have not been seen within the past year:
You will need to schedule an appointment prior to having a prescription refilled. This allows our doctors to evaluate your current situation and provide you with the most recent developments for your condition.
If you do not have refills or need a new prescription:
If you have been in our office within the past year, refill requests will be reviewed by your dermatologist. All requests for prescription refills are processed within 2 business days. Please complete the following form.
